Interview with Dr. Ignacio Ordiz García
New implants: hylanSeS & Plinest
Dr. Ignacio Ordiz has a degree in Medicine and Surgery from the University of Oviedo, with a Masters in Aesthetic Medicine by SEME, specialist degree in Homeopathy (Valladolid University) and graduated in Mesotherapy (University Victor Segalen, Bordeos).
He has extensive experience in the field of Aesthetic Medicine, and therefore in anti-aging strategies like revitalization and mesotherapy or the use of botulinum toxin and hyaluronic acid implants.
In Mediderma we have recently commercialized two product lines based on skin revitalization and wrinkle filling: plinest and the hyaluronic acid implants hylanSeS.
We will start with plinest, of which Dr. Ordiz is one of our key users. What is your opinion on Plinest?
A few years ago a product very similar to plinest disappeared from the French market. This was a real misfortune because the product gave us very good results in revitalizing treatments and alopecia. Thanks to Mediderma we now have a similar product available. I would even say that this product is much improved since the research from has run in our favour since the time that the other product was taken off the market. For me it is an essential product for facial revitalization or results with alopecia, stretch marks, and all of these skin ailments that need an extra supply of polynucleotides.
What other products are used in revitalization?
The ideal complement to polynucleotides would be non reticulated hyaluronic acid as well as homeopathic products composed of cutis, connective tissue and stretched muscle. Multivitamin Complexes may also be a good supplement, and of course, organic silicon but always a salicylate formula and not manorunato which can not be injected.
Do you think you have "competition"?
I do not know of another product covered under the CE and Authorized by the Ministry of Health and Consumer Affairs for use as a Class III implant.
When can you begin to see its effects?
Throughout the revitalization process, results are variable depending on many factors such as the initial state of the skin, the patient's age, sun exposure, and concomitant therapies. But I can say that results are often rapid during the first 2 or 3 sessions, especially when associated with other stimulating therapies such as certain peelings, pulsed light, or laser therapies.
Is it well tolerated by patients?
Generally yes, now we always use a mesotherapy gun for injections. Some guns that are great for this task are the U225 and Dr. Injector that now and allow us to calibrate the pressure on the plunger of the syringe to adjust the viscosity of the product. In the case of manual injections the patient may be less tolerant of the needle prick, but with a good technique is not painful at all.
Does it meet the patient’s expectations?
My patients and I both love the ease of handling, and the ability to combine the treatment with other treatments, and the degree of "loyalty" we get at the time to repeat a treatment.
Our second product line is hyaluronic acid implants. In this area we have a lot of competition. What do you focus on when choosing one implant or another?

All these concepts are important when choosing one product over another. In my case, I have focused on the composition of the range, the cross linking agents, and in the technology that allows us to obtain a molecule size that is suitable and standard.
Do you consider it important to know the manufacturing process for implants?
It is clear that any doctor should worry about how the products they are going to use in their patients are manufactured, and not only focus on the publicity that a commercial can bring. For me personally I am very interested in the how the product is manufactured, and that it has particles of an appropriate uniform size to ensure optimal results without causing inflammatory reactions and movements.
What would you stress about our hylanSES implants? Can you tell your experience? Are they well tolerated?
Well, I think it is clear that besides the composition and the association of hyaluronic acid being with high and low density, the manufacturing process and the agent reticulante are certainly unusual in the hylanSeS line. My experience with the whole line is good; I have not encountered cases of intolerance or excessive inflammatory reactions.
Is the injection technique used by the professional important?
Of course. This is key to obtain the proper result. We have to adjust the selected product for the wrinkles or irregularities that we treat. Each patient is unique; therefore, these parameters are critical and are the highlight when we do practice workshops.
What precautions should we take at the time to administer Hylanses?
When do implants we always prefer to fill in a progressive manner, in consecutive sessions. I always tell my patients that it is always better to arrive "short" than to over correct. It is also important to "shape" of the implant after application by a massaging it with your fingers to give it a ufiform distribution in the implanted area, but we should be careful not overdo it to prevent unwanted inflammatory reactions. We can apply ice 3-4 times a day for about 5-10 minutes, always protecting the skin from direct contact with the ice with a towel or gauze: this is particularly useful if small bruises emerged during the intervention. Also we must always obtain an informed consent.
We are all frightened by the appearance of granulomas with filling implants. Have you had any cases of this?
The appearance of nodules (more than granulomas) with hyaluronic acid implants is always unlikely with an appropriate application. However, it is not infrequent for small deposits of material to appear. These disappear in a few days with gentle massage of the area by the patient himself.
